Confidentiality

Care in Crisis adheres to professional standards of confidentiality. This means that information will not be
disclosed to anyone outside Care in Crisis.

The principle of confidentiality can only be superseded in an extreme situation, in which there is a high
level of concern about serious harm to the client or others. Your counsellor will always discuss this with
you and inform you if a third party needs to be consulted.

Notekeeping
Notes are kept routinely of all counselling sessions. These act as a memory aid to the counsellor and are
kept in a locked cabinet on the premises of Care in Crisis in between sessions. The notes are coded with
a client ID number and first name of the client. The referral form containing personal information such as
full name, address, date of birth are filed separately from the client file for ongoing counselling sessions.

Care in Crisis will not release original or copies of client notes to an external agency or individual but if
you wish to have your attendance at counselling confirmed to a third party we will, with your consent,
provide a general statement regarding the number of sessions attended. The only exception to this policy
is if Care in Crisis is summoned to release client notes by a Court of Law.

As the client you may have sight of your notes on the premises of Care in Crisis at any time without
removing them. If your counselling has finished and you would like to see your client notes you can
request to do this by writing to the Director of Counselling at Care in Crisis, 41 Union Street, Lurgan,
Craigavon, BT66 8DY. You will be given an appointment to come into the Centre and have the
opportunity to read your notes on the premises.
